Documentation to be submitted by the sponsoring department to OIT:

• Employee-Independent Contractor Classification Checklist
• Check Request Form
• Foreign National Information Form (optional for payment; mandatory for treaty benefits)
• Social Security Card or ITIN (optional for payment; mandatory for treaty benefits)

Forms to be completed and signed through the OIT:

• Form W-8BEN
• Form 8233 & Certification Statement (if tax treaty provision exists)
